The undersigned hereby makes application to Heritage United Methodist Church (HUMC) for the use of the facilities listed above. The undersigned warrants that the applicant will exercise the utmost care in the use of HUMC premises and property, and will make good any damage to said premises and property which arises during the applicant’s use thereof, no matter what the cause.

Families and guests must remain with your group at all times.
Children must be supervised by an adult who is part of your group.

In the event HUMC must cancel your event, advance notice will be given as soon as possible. The use of the facilities must meet the Mission Statement of HUMC and the Social principles of the United Methodist Church. Approval for the use of the facilities will be at the discretion of the Pastor and the Board of Trustees. The cost and payment schedule associated with the use of the facility will be agreed upon and paid one week in advance of the event to the HUMC office.

 

To the extent the law allows, the applicant agrees to indemnify and hold harmless Heritage United Methodist Church, its agents, and employees, from all claims, including injuries to persons, and/or property arising, directly or indirectly, out of such use.

 

This application and subsequent approval, if given, are recognized for twelve months. Renewal of the application should be made for the next year at least 60 days before expiration to ensure adequate time for the approval process. HUMC must always have a current Contact Person with a telephone number and email on file. If any information on this application changes, including the Contact Person, a revised application must be submitted. The information change in the Contact Person will not require the Board of Trustees' approval, but all other changes will be resubmitted for approval.

 

Note: The Americans with Disabilities Act requires HUMC to ensure that all independent organizations with which we work comply with ADA regulations. Your signature indicates that your organization complies with ADA to the best of the applicant’s knowledge.

      Disciple Center -Gym              $40/hr       or       $500/15-hour day, ex. 7 AM - 10 PM

                                             

                                                        $800/day for 24 hrs         

 

                                                        $2000/week @15 hrs/day

      Fields - Upper OR Lower        $25/hr
